In a recent example of effective collaboration across community services, Civitas worked alongside Queensland Positive People to support “Jade” a participant requiring urgent and intensive assistance following hospital discharge.
“Jade”, who had experienced persistent homelessness, unmanaged health conditions, and significant barriers to stable housing, faced the prospect of returning to street homelessness immediately after surgery. With a history of resilience, and a growing openness to change, Jade represented both the complexity and potential that Supported Living strives to address.
Julian, a Case Worker at QPP, reached out to Nathan Johnson, Founder of Civitas, for assistance in identifying a supported housing option suitable for Julie’s needs. Nathan and the Civitas team responded swiftly, offering practical guidance and facilitating a referral to Clayfield House, one of Civitas' well-regarded supported accommodation facilities.
Through close coordination, the necessary arrangements were made to support Julie's transition. Key considerations included her preference for a Northside location, her eligibility for Disability Support Pension (DSP), and the need for structured financial management through Centrepay. Despite not currently being on the NDIS, Julie’s case was identified as a future candidate for access to the scheme, with ongoing case management to support this pathway.
When Julian visited Clayfield House, he was struck by the positive environment established by the team. Staff members Daniel and Esther demonstrated a high level of professionalism and care, creating a communal setting where residents felt welcome and supported. The positive culture within the house reflects Civitas' commitment to resident wellbeing, providing not just accommodation but a sense of belonging and stability.
Jade’s acceptance into the program marked a pivotal step toward her recovery and reintegration into the community. With the hospital extending her discharge by one day to accommodate the transition, Julie was able to move into Clayfield House smoothly, supported by familiar case management contacts and a carefully considered housing plan.
