Eligibility for in-home aged care services is determined through a My Aged Care assessment conducted by the Australian Government.
To qualify, you generally must:
- Be aged 65 years or older (or 50 years or older for Aboriginal and Torres Strait Islander people)
- Have difficulty managing everyday tasks such as bathing, cleaning, or cooking
- Be living at home in Australia, including Brisbane, Logan, Caboolture, or the Sunshine Coast
If you’re unsure of your eligibility, the Supported Care team can help you navigate the My Aged Care referral and assessment process, making it simple to start services through the Support at Home program.

Under the Support at Home program, the amount you pay depends on three factors:
1. Your approved funding level (Levels 1–8)
2. Whether you were approved for aged care before September 2024
3. The type of services you receive
If you were approved before September 2024, you will not be required to make a co-contribution for government-funded services.

If you’re experiencing financial hardship, you may be eligible for government assistance to reduce or remove aged care fees.
The Australian Government offers financial hardship assistance for eligible people who cannot afford their aged care contributions. This can include:
- Reducing or waiving care fees
- Additional government supplements paid to your provider
- Support for essential care where finances are limited
Hardship assistance is assessed by Services Australia and is based on your income, assets, and personal circumstances.

The Support at Home program is a new Australian Government initiative that provides funding for older Australians to receive personalised care and support at home rather than moving into residential care.
It combines several existing programs — including Home Care Packages, the Commonwealth Home Support Programme (CHSP), and Short-Term Restorative Care — into one simplified model from November 2025.
Under this program, eligible people can access services such as:
- Personal and domestic assistance
- Transport and community participation
- Allied health and clinical care
- Home modifications and equipment
Supported Care delivers services under the Support at Home framework across Brisbane, Logan, Caboolture, and the Sunshine Coast, helping clients stay safe, connected, and independent at home.
